name is __________ I am a Registered Nurse here at Mosaic Medical. I am calling on behalf of your doctor/provider
___________ . Your doctor/provider has
chosen you to participate in our remote patient monitoring program so he/she
may better manage your healthcare.
How it
works: This is a free program offered at no cost to
the patient. We lend you a monitor that you will use to check your blood
pressure, weight, and oxygen saturation. You would check your blood pressure or
weight (what your doctor orders) one time per day, 5-7 days per week depending
on what your doctor orders. The information is sent via Bluetooth to a secure,
confidential site where it will be reviewed by your RN case manager which will
be me. If there are any questions, concerns, or problems I will contact you. If
there is anything out of range, it will be communicated to your doctor who will
determine if there needs to be changes made to your medications or your health
care. We have a team care assistant that can also assist with your needs.
Your
doctor/provider and I think you would be a perfect candidate for this program
and we would like the opportunity to help you improve your health and the care
you receive.
